As already mentioned, the questionnaire is an anonymous document. In fact, researchers are not interested in this case, the name of the Respondent, but each Respondent questionnaire is a document. who reveals some of his personal side. Not always nice, if in writing, expressing some of my thoughts, estimates, even of an intimate nature, and do not know who will read it. I myself had someone else to fill the questionnaire and I must admit that the feeling wasn't always very pleasant. If anyone of the readers filled out questionnaires or questionnaire, it is likely that he will agree with me. The guarantee of anonymity has an important psychological value, so the user always specifies: "the results of the study will be used for research purposes only...", "Name not required".
But here, too, need to be correct. Not to write categorical: "Name unknown" or "Us Your name is not interested and only want answers." Own name is one of the most expensive things in human life, and it is unlikely he will like it, if we emphasize that we are not interested in his name, we are generally not interested in anything except the answers. And then, why not give him the opportunity to write his / her name? Some respondents sign their questionnaires.
The second important part of the user - description of the basic rules of filling out the questionnaire. Usually in this part there are three points:
-- drawn the Respondent's attention that he has to choose from a range of alternatives the one which is most suitable to him;
-- what can he finish the answer, if none of the suggested answers does not suit him, for this and left free space;
